The ZIP code for the subject property physical address must be either 5 digits, or 5 digits, a hyphen, and 4 digits (ZIP+4).
Fatal — a single unresolved Fatal makes the entire submission come back NOT SUCCESSFUL (UCDP Hard Stop 401).
What triggers UAD1005
The published rule logic, verbatim:
If PostalCode is not in the required format (5 digits, or 5 digits, a hyphen, and 4 digits)

How to fix it
On the URAR, this lives at: Subject Property → Physical Address (%PV%). Complete or correct that field in your form software, regenerate the package, and re-check before submission.
Because this is a Fatal edit, do not submit until it is resolved — one Fatal bounces the whole package.
